David Cottengim Special Assistance:
NOVA established the David Cottengim Special Assistance Fund in 1999 as a program to provide emergency aid to individuals diagnosed with hemangioma or vascular malformation. The fund covers, transportation, lodging, and other non-medical assistance not covered by health insurance during acute or emergent medical care.
This is an emergency aid grant program for individuals that will not be able to receive medical care without assistance.
To qualify for assistance from the fund the patient/family must complete and return to NOVA, the application and a signed financial need verification form.
The applicant of this assistance must be the patient or the parent or legal guardian of the patient. Verification of need must come from a professional person such as a social worker, clergy, physician, accountant, attorney, employer or Executive Officer of a charitable organization such as the American Red Cross or incorporated church.
Caitlin Rose Moncrief Memorial
NOVA established the Caitlin Rose Moncrief PHACES Research and Assistance Memorial at the request of Tom and Michelle Moncrief. The program is in memory of their daughter Caitlin who was diagnosed with the PHACES Association and other complicated medical issues. This fund specifically provides for research and or assistance to individuals diagnosed with PHACES. The fund covers, transportation, lodging, and other non-medical assistance not covered by health insurance while undergoing treatment or in emergency situations. To qualify for assistance from the fund the patient/family must complete and return to NOVA, the application and a signed financial need verification form.
