NOVA Privacy Policy
The Health Insurance Portability Act of 1996 (HIPPA) is a federal program that requires that all medical records and other individually identifiable health information used or disclosed in any form, whether electronically, on paper or orally be kept properly confidential. This Act gives you, the patient or legal parent/guardian of a minor child, significant rights to understand and control how your health information is used. HIPPA provides penalties for covered entities that misuse personal health information.
In compliance with HIPPA guidelines, NOVA adheres to a strict policy of confidentiality. NOVA will not release identifiable medical information you have provided to us to anyone other then the patient or parent/legal guardian of a minor child without expressed written consent. Photographs will not be used in any print media for any reason without prior written consent. Personally identifiable information is stored in a secure manner.
NOVA may create and distribute de-identified information by removing all references to individually identifiable information. We may do this in one of the following but not limited to ways: NOVA may compile data or our organizational reports, newsletter or other documents used by our organization. We may use de-identified information in correspondence with physicians as we attempt to assist a patient or a parent of a patient.
Any other uses or disclosures will be made only with your prior written authorization.
Message Board
NOVA operates a public message forum at http://www.novanews.org/forum/default.asp The message forum is a public board which can be viewed by anyone with access to the internet. Posting to the message board requires joining the board with an acitve email account. NOVA can not maintian confidentiality to postings on the message board forum. Advertising and Financial Support
This web site is operated and maintained by the National Organization of Vascular Anomalies Board of Directors. NOVA is a 501c3 non-profit organization. Our efforts, as an educational resource and advocacy group for individuals affected by hemangioma and vascular malformation, are funded by contributions from individuals. NOVA does not host or recieved funding from advertisers.
Charitable Status and Tax Identifications
The National Organization of Vascular Anomalies is a non-profit organization. In April 1999 the Internal Revenue Service issued a dertermination letter that recognized NOVA as exept from federal income tax. NOVA is exepmt undersection 501(c)(3) of the Internal Revenue Code. NOVA is classified as a public charity under sections 509(a) (1) and 170 (b)(1)(A)(vi) of the Internal Revenue Code. Contributions to NOVA are tax dedcutible. NOVA is qualified to receive tax dedcutible bequests, devises, and transfers of gifts under section 2055, 2106 or 2522 of the Internal Revenue Code. Federal Identification Number 56-2054522
